play.connect.thrive provides high-quality early childhood intervention supports via a play-based approach that builds upon your child’s strengths and interests to promote growth and learning. Our Early Intervention Specialist Key Worker will work with you and your child to support their individual goals.
We are experienced in supporting children’s cognitive, communication, social, emotional, fine and gross motor development. We are also passionate about supporting children’s independence and play skills.
We offer our services to self-managed or plan-managed NDIS participants in Hervey Bay and across the Fraser Coast.
Our services are also available to self-funded families.

Current research tells us that best practice in Early intervention takes on a family-centred, inclusive approach delivered through the ‘key worker’ model.
Key Workers are early intervention specialists who are the key point of contact for a child and their family. Our key worker provides high-quality early intervention supports via a play-based approach that builds upon your child’s strengths and interests to promote growth and learning.
Your key worker will work with you and your child to support their individual goals including:cognitive development
communication (including AAC & devices)
social and emotional
fine motor development
gross motor development
Your Key Worker will visit your child weekly or fortnightly in their natural settings, wherever you need support. This can be at your home, childcare/school and in the community. Your Key Worker will provide support across all the areas you require to reach your goals; they will work with, add to and compliment the learnings your child will receive through clinical therapy. We call this a ‘transdisciplinary’ approach.
We can help you connect with therapists as needed or we can continue to work with the ones you have. -
Your Key Worker will provide you with coaching and mentoring to develop skills and strategies that promote your child’s development. Just some of the coaching we can provide includes strategies to support
positive behaviour
early language and pre-literacy skill development
engaging your child in play
encouraging independence with self-care skills.
Your Key Worker will also work with you to incorporate strategies from your child’s current therapy programs into your everyday routines and activities to make ‘therapy homework’ more manageable.

Group learning is widely recognised to be crucial to every child’s successful development. Children typically make use of groups to practice their communication skills, make friends, observe their peers and learn by example. We offer the opportunity for children with similar social or functional needs to come together, form connections with others and practice their skills.

A transition to school that is well facilitated and supported can contribute to successful child outcomes, especially for children with disability/developmental delay. Our Key Workers can provide information, practical and emotional support to assist with making the transition to school as smooth as possible. This may include information about the range of school options available relevant to your child, planning for the transition, creating resources to support them, how to advocate for your child, and attending transition meetings.

Ready, Set, Prep is a tailored program to support a child in the year prior to entering Prep, and can support areas of development including:

social & emotional
fine motor
gross motor
Ready, Set, Prep can take place in a group or individual sessions, depending on the needs of the child/ren.
